Version v1.0

Last Updated: September 2026

Covalent Clinical Research (“we,” “our,” or “us”) is committed to protecting the privacy and security of your health information. This HIPAA Notice describes how we handle Protected Health Information (“PHI”) that you voluntarily provide through our website, interest forms, communications, or participation in clinical research activities.

What Is Protected Health Information (PHI)

PHI includes any information that relates to your health, medical conditions, medications, symptoms, treatment history, or potential participation in clinical studies. By submitting information to us or engaging with our clinical research services, you acknowledge and understand the practices described in this Notice.

How We Collect and Use PHI

We collect PHI only when you choose to provide it, such as when you complete an interest form or communicate with our clinical team. We use PHI solely for purposes related to clinical research, including screening, determining potential eligibility, communicating with you about research opportunities, conducting study activities, and meeting regulatory requirements. PHI is never used for marketing, and we do not sell or disclose PHI for non‑research purposes.

How We Protect PHI

Covalent Clinical Research protects PHI through administrative, technical, and physical safeguards designed to prevent unauthorized access, use, or disclosure. These safeguards include secure data storage, encryption, access controls, staff training, and HIPAA‑compliant communication practices. Only authorized personnel involved in clinical operations, regulatory compliance, or patient communication may access PHI.

When We May Share PHI

We may disclose PHI only when necessary to support clinical research activities. This may include sharing information with clinical investigators, research staff, Institutional Review Boards (IRBs), regulatory agencies such as the FDA, sponsors or CROs involved in study oversight, or healthcare providers when appropriate. Any disclosure is limited to what is required for research or compliance, and we do not disclose PHI for commercial or promotional purposes.

Your Rights Under HIPAA

You have rights under HIPAA regarding your PHI. These rights may include requesting access to your PHI, requesting corrections to inaccurate information, requesting restrictions on certain uses or disclosures, requesting confidential communications, or requesting an accounting of disclosures. You may also request deletion of PHI unless retention is required by clinical research regulations. To exercise your rights, you may contact us at admin@covalentclinicalresearch.com.

We may update this HIPAA Notice periodically to reflect changes in our practices or legal requirements. The “Last Updated” date indicates the most recent revision. Continued use of our website or participation in our research activities indicates acceptance of any updated terms.
